First-in-Human Prospective, Observational, and Comparative Clinical Study of Simultaneous Invasive and Non-Invasive Intracranial Pressure Pulse Wave Monitoring

by Indre Lapinskiene, Edvinas Chaleckas, Vilma Putnynaite, Laimonas Bartusis, Yasin Hamarat, Aidanas Preiksaitis, Mindaugas Serpytis, Vytautas Petkus, Saulius Vosylius, Arminas Ragauskas

Monitoring intracranial pressure (ICP) dynamics is critical for the management of traumatic brain injury, stroke, other neurosurgical conditions, and cerebral blood flow autoregulation; however, invasive ICP monitoring carries risks such as infection, hemorrhage, and sensor zero drift. Increasing evidence suggests that ICP waveform morphology provides clinically relevant information beyond mean ICP value alone. In this first-in-human prospective comparative clinical study, we evaluated the feasibility and accuracy of a novel, fully passive, non-invasive ICP pulse waveform monitoring system (Archimedes 02) based on the detection of eyeball mechanical movement. Fifteen intensive care unit patients (6 males, 9 females; mean age 57.1 ± 18.8 years) with clinically indicated invasive ICP monitoring or external ventricular drainage were enrolled. Three-minute monitoring sessions were performed to simultaneously acquire non-invasive ICP pulse waveforms, invasive ICP waveforms, and invasive radial artery blood pressure (ABP) waveforms. Averaged waveforms were derived for each patient and compared graphically and using correlation analysis. Non-invasive ICP pulse waves recorded with Archimedes 02 showed a strong correlation with invasive ICP waveforms (R¯ = 0.965). In contrast, correlations between non-invasive ICP and ABP waveforms (R¯ = 0.699), as well as between invasive ICP and ABP waveforms (R¯ = 0.749), were lower. These findings indicate that the non-invasive signal primarily reflects ICP dynamics rather than arterial blood pressure. This novel non-invasive ICP monitoring approach has the potential to enhance neurocritical care, particularly in settings where invasive monitoring is impractical or unavailable. Further validation in larger and more diverse patient populations is warranted.

  • Optimal head rotation angle for safe right internal jugular vein catheterization using out-of-plane approach: an ultrasonography study

by Saulius Vosylius, Akvile Sabestinaite, Raimundas Stasiunaitis

Aim: The most common complication following internal jugular vein catheterization is the puncture of the common carotidartery. We aimed to find an optimal head rotation angle for safe vein catheter insertion while minimizing the risk of arterypuncture.Materials and methods: Eighty-two patients admitted to the intensive care unit were included in the prospective study. Ultrasound examination of the neck vessels on the right side was performed in the neutral position and at a head rotation of 15°, 30°, 45° and 60° to the left. Internal jugular vein and common carotid artery puncture angle, overlapping angle of vein by artery and vein safe puncture angle were evaluated. Results: The internal jugular vein puncture angle increased with head rotation from the neutral position to 30° and 45° and was largest at 60° (p<0.001 for all). The overlapping angle increased significantly at 45° and 60° rotation compared with the neutral position (p<0.001 for all). The vein safe puncture angle was highest at 30° and significantly different from the neutral position and 60° (p=0.003 and p=0.013, respectively). Conclusions: When performing right internal jugular vein catheterization without ultrasound guidance by using an out-of-plane technique, the head should be rotated at 30°, because the overlapping angle increases with further head rotation and can increase the risk of artery puncture.

by Viktorija Černiauskienė, Monika Čiplytė, Saulius Vosylius

Viktorija Černiauskienė, Monika Čiplytė, Saulius VosyliusVilniaus universiteto Anesteziologijos ir reanimatologijos klinika, Vilniaus greitosios pagalbos universitetinė ligoninė,Šiltnamių g. 29, LT-04130 VilniusEl paštas: saulius.vosylius@gmail.com Įvadas / tikslasDonorinių organų poreikis gerokai viršija atliktų organų transplantacijų skaičių. Dažniausios priežastys, dėl kurių potencialūs donorai netampa efektyviais donorais, yra donoro artimųjų prieštaravimas donorystei, medicininės kontraindikacijos, logistikos problemos, neadekvatus potencialių donorų gydymas iki eksplantacijos operacijos. Šio straipsnio tikslas yra apžvelgti naujausius medicinos mokslo laimėjimus atliekant potencialių organų donorų priežiūrą reanimacijos ir intensyviosios terapijos skyriuose. Metodai ir rezultataiIšliekant žymiam atotrūkiui tarp organų pasiūlos ir poreikio, būtina kuo efektyviau panaudoti esamus resursus, daugiau dėmesio skiriant tinkamai potencialaus organų donoro intensyviajai terapijai. Dėl smegenų mirties įvyksta sunkūs daugelio organizmui svarbių funkcijų sutrikimai: kraujotakos ir kvėpavimo sistemų, endokrininiai, elektrolitų balanso sutrikimai, hipotermija, koagulopatija ir intensyvus sisteminis uždegimo atsakas. IšvadosPasirinkta tinkama intensyviosios terapijos taktika galėtų padidinti transplantacijai tinkamų organų skaičių, išlaikyti geresnę jų funkciją po transplantacijos. Many reasons may determine this inadequacy, such as family refusal, medical contraindications, logistics problems and inadequate management of the organ donor. The aim of the study was to present the recent achievements of medical practice in the management of organ donors in intensive care units. Methods and resultsWhile the discrepancy between the number of organ donations and transplantations persists, it is essential to use available resources more effectively, paying more attention to the intensive care management of the organ donor. Many physiological changes follow after brain death, such as cardiovascular and pulmonary dysfunction, endocrine and homeostasis disturbances, hypothermia, coagulopathy and an enhanced inflammatory response. ConclusionsOptimal intensive care could increase the number of organs available for transplantation and improve their function after it. Keywords: brain death, intensive care units, tissue and organ procurement, guidelines as topic

    Sepsio diagnostika

    by Saulius Vosylius

Reikšminiai žodžiai: sepsis, diagnozė, infekcija, sisteminio uždegimo atsako sindromas, dauginis organų nepakankamumas, uždegimo mediatoriai Diagnosis of sepsis Saulius VosyliusClinic of Anaesthesiology and Intensive Care, Vilnius University,Šiltnamių str. 29, LT-04130 Vilnius, LithuaniaE-mail: saulius.vosylius@gmail.com Background / objective The diagnosis of sepsis has always been an object for discussion. Early diagnosis and therapy are important in the management of critically ill patients. Methods Diagnostic criteria for sepsis and related states are still not well defined. The International Intensive Care Society conference held in 1991 proposed and later in 2001 revised the definitions of sepsis and related conditions. Results Infection is a pathologic process caused by the invasion of normally sterile tissue or fluid or body cavity by pathogenic microorganisms. Sepsis is defined as systemic inflammation caused by infection. Clinical signs related to sepsis are not specific to infection. To diagnose and monitor sepsis and to guide therapy, currently it is necessary to employ a combination of parameters. The diagnosis of sepsis is supported by clinical criteria and confirmed by laboratory parameters and microbiological data. Severe sepsis are confirmed when sepsis is complicated by tissue hypoperfusion and organ dysfunction. Septic shock refers to a state of acute circulatory failure characterized by persistent arterial hypotension unexplained by other causes. Conclusions Early diagnosis of sepsis is crucial in providing adequate therapy and improving the outcome. The search for the parameters of the diagnosis and monitoring the progress of sepsis is continuing. It should be specific for the diagnosis of infection, reflect the systemic inflammation, and correlate well with inflammation intensity. Key words: sepsis, diagnosis, infection, systemic inflammatory response syndrome, multiple organ failure, inflammation mediators
