Deep learning for odontoid fractures: the critical role of patient-level partitioning and Grad-CAM assessment in model generalization

by Yunus Emre Kıymaz, Şakir Taşdemir, Burak Gezer, Kübra Uyar Pınar

Missed diagnoses of odontoid fractures on Computed Tomography (CT) scans can lead to severe morbidity. While Deep Learning (DL) shows promise, many studies report inflated performance metrics due to hidden data leakage (image-level splitting). To evaluate six Convolutional Neural Network (CNN) architectures under a strict zero-leakage protocol for odontoid fracture detection, validating diagnostic reliability and anatomical focus using Explainable AI (XAI). A retrospective cohort of 540 cervical spine CT images from 96 patients was evaluated under a strict zero-leakage protocol. Data was partitioned at the patient level into three disjoint subsets: Training (60 patients, 326 images), Validation (16 patients, 92 images for early stopping), and an independent held-out Test set (20 patients, 122 images touched only once for final evaluation). Six CNN architectures were benchmarked. Statistical robustness was validated via patient-level bootstrapping ( N = 1000 iterations) with Holm-Bonferroni family-wise error rate correction, paired with Grad-CAM visual interpretability. On the untouched test cohort, DenseNet121, GoogLeNet, and ResNet18 achieved similar, leading discriminative performance with AUCs of 0.830 (95% CI: 0.718–0.933), 0.829 (95% CI: 0.714–0.929), and 0.829 (95% CI: 0.697–0.934), respectively, with DenseNet121 demonstrating a balanced diagnostic profile (Accuracy: 73.8%, Sensitivity: 73.0%, Specificity: 75.8%). In stark contrast, SqueezeNet 1.1 and VGG19 suffered complete collapse to majority-class prediction (Specificity: 0.0%) under the evaluated training configuration. Pairwise bootstrap hypothesis testing confirmed that DenseNet121 showed a borderline statistically significant difference over SqueezeNet 1.1 after multiple testing correction ( $$\:{p}_{\text{raw}}=0.0090$$ , Holm-adjusted $$\:p=0.0450$$ on $$\:n=20$$ patients), while differences among DenseNet121, GoogLeNet, and ResNet18 remained statistically non-significant ( $$\:{p}_{\text{adj}}=0.9840$$ ). Architectures incorporating deep feature reuse or residual connections exhibit robust generalization under strict patient-level isolation, whereas compact models and unregularized heavyweight architectures can struggle under small-sample constraints. Rigorous zero-leakage methodology and qualitative XAI assessment are essential prerequisites prior to clinical translation of spinal CAD models.

Early Recurrence of Lumbar Disc Herniation: A Retrospective Analysis of the Role of Discectomy Volume and Other Influencing Factors

by Derya Karaoğlu Gündoğdu, Burak Gezer, Ender Kökteki̇r, Hakan Karabağli

Objective: The aim of this study is to conduct a detailed examination of cases of "Early Recurrence" encountered following lumbar disc herniation, and to compare the data obtained with findings in the existing literature. Materials and Methods: This retrospective study was conducted by analyzing the data of 856 patients who were operated for recurrent lumbar hernia in our hospital between 2012 and 2022. By expanding the definition of "early relapse" in the literature, inclusion and exclusion criteria were determined, and as a result, 43 patients who met the criteria were included in the study Results: The severity of symptoms was assessed based on patients' Visual Analog Scale (VAS) scores at baseline and during subsequent periods. Patients requiring reoperation within the first 12 months were divided into two main groups: those undergoing reoperation within the first 6 months and those undergoing reoperation between 6-12 months. However, no statistically significant difference was found between the two groups in terms of age, gender, comorbidities, and other determinants. Notably, patients undergoing surgery within the first 3 months were observed to be of advanced age. Conclusion: When comparing the data obtained from analyzing patients with "early recurrent lumbar disc herniation" with findings shared in the literature, we found that similar variables were not significant in our series. An interesting observation is that both full endoscopic and microscopic methods have similar early recurrence rates. This is particularly noteworthy because full endoscopic discectomy usually involves sequestrectomy, and the volume of the disk removed is typically minimal. Hence, our study suggests that the volume of the disk removed during surgery may not be as influential in the development of early recurrence as previously thought. These findings provide an important foundation for future research.

    Pediatric Posterior Fossa Epidural Hematomas

by Burak Gezer, Ahmet Selim Karagoz, Mert Şahi̇noğlu, Hakan Karabağli

Aim: Posterior fossa epidural hematoma (PFEDH) in the pediatric age group is a very rare condition and the treatment method is still a matter of debate. The aim of this study is to evaluate the observation results in the management of pediatric patients with PFEDH in our tertiary care hospital and to investigate the importance of the relationship of the hematoma with the cerebral venous sinuses, which has not been investigated before in the literature, in the treatment decision. Materials and Methods: This is a retrospective study conducted at Selcuk University, Faculty of Medicine. All patients (≤ 17 years) diagnosed with PFEDH between January 2010 and May 2022 were included in this study. Demographic data, clinical signs, trauma type and symptoms at presentation, CT findings, type of treatment, and outcomes were collected. CT findings including hematoma thickness, hydrocephalus, presence of fourth ventricular compression, relation with cerebral venous sinuses and other associated brain injuries were evaluated. Results: The patient group consists of two girl and six boy. The most common cause of PFEDH was a fall from a height resulting in a blow to the back of the head in four patients. Vomiting was the most frequent presenting symptom. Four patients had a relation between cerebral venous sinuses and hematoma, and two of these patients underwent surgical treatment Conclusion: In addition to criteria such as hematoma thickness, GCS, hydrocephalus, and compression of the fourth ventricle, we determined that the relationship of hematoma with venous sinuses is a criterion to be evaluated.

  • Acute epiglottitis is a rare clinical presentation of coronavirus disease 2019: a case report

by Burak Gezer, Hakan Karabagli, Mert Sahinoglu, Ahmet Selim Karagoz

In humans, coronavirus disease 2019 (COVID-19) has a variable presentation ranging from mild self-limiting respiratory tract infections to severe acute respiratory distress syndrome. Methods: We present the case of a patient who developed acute epiglottitis after surgery for an intracranial tumour and was subsequently diagnosed with COVID-19. A 58-year-old female patient developed acute-onset respiratory distress on day 6 after intracranial surgery. Neck computed tomography revealed near-total airway obstruction due to severe edoema of the epiglottis and periepiglottis. The patient’s SARS-CoV-2 polymerase chain reaction test was positive. Viral respiratory tract panel and sputum and blood cultures were negative. She completed the antiviral and antibacterial treatment regimens recommended by the chest disease department. We conclude that epiglottitis can be an unusual manifestation of COVID-19.

  • Evaluation of our neuronavigation assisted biopsy results according to lesion location and size

by Mert Şahi̇noğlu, Derya Karaoğlu Gündoğdu, Burak Gezer, Mehmet Ali Kizilay, Hakan Cebeci̇, Pınar Karabağli, Ender Kökteki̇r, Hakan Karabağli
