Modifiable Factors Underlying Caregivers’ Psychological Support Needs in Pediatric Disability: Through the Lens of Psycho-Behavioral and Social–Environmental Interactions

by Hongying Zheng, Mei Sun, Anni Wang, Qi Tang, Yaping Zhang, Jun Lu, Gang Chen

Background/Objectives: Childhood disability is a major stressor for caregivers. There are many problems and challenges in relation to satisfying the psychological support needs of caregivers. This study aims to explore the modifiable psycho-behavioral and social–environmental factors of psychological support needs and need satisfaction, their interaction effects, and their configuration paths. Methods: This was a cross-sectional survey of 363 caregivers using instruments such as the psychological support needs subscale of the Care Needs Assessment Tool for Children with Disabilities. Multivariable logistic regression with interaction terms and crisp-set qualitative comparative analysis were performed. Results: The overall rate of psychological support needs was 78.29%, and the overall need satisfaction was 49.94%. Multivariable logistic regression analysis showed that the caregiver’s need was mainly influenced by anxiety, while need satisfaction was primarily predicted by public policy support and social attitudes. There was an interaction effect between caregiver anxiety and social attitude on need satisfaction. Qualitative comparative analysis indicated that six paths were identified as potentially leading to high levels of psychological support need, while five paths were associated with low levels of need satisfaction. Conclusions: The psychological support need rate among caregivers was high, and the level of need satisfaction was low. There were significant differences in psychological support needs and need satisfaction among different psycho-behavioral and social–environmental characteristics. It is recommended that caregivers with negative emotions be given more attention. The enhancement of social attitudes and the adoption of more supportive policies will contribute to the improvement of need satisfaction.

Depression and Associated Factors among Family Caregivers of Children with Disabilities: Analysis of Intergenerational Differences

by Cong Xia, Ting Wei, Qi Tang, Hongying Zheng, Gang Chen, Jun Lu

Family caregivers of children with disabilities might face high risks of depression, whereas the existing literature focused more on parents neglecting grandparents. This study investigated 380 parents and 108 grandparents of children with disabilities to identify depression and associated factors. Descriptive statistics, Chi-square test, Mann–Whitney U test, and multivariable logistic regression were performed to describe the participants’ characteristics and risks of depression and identify significant factors. Results showed that parents (35.5%) had higher risks of depression than grandparents (32.4%), but statistical differences were not found. Children’s sleep problems (AOR = 1.751, 95%CI = 1.019, 3.008), harmonious family relationships (AOR = 0.694, 95%CI = 0.569, 0.846), and better barrier-free construction (AOR = 0.742, 95%CI = 0.568, 0.970) were significantly associated with depression among parents. As for grandparents, higher education (AOR = 4.108, 95%CI = 1.526, 11.057) and caring for children who experience frequent mood swings (AOR = 2.242, 95%CI = 1.161, 4.329) were associated with higher risks of depression. Further, house ownership (AOR = 0.167, 95%CI = 0.031, 0.887), higher family cohesion (AOR = 0.545, 95%CI = 0.297, 1.000), and better barrier-free construction (AOR = 0.401, 95%CI = 0.185, 0.869) were associated with lower odds of depression. Therefore, both parents and grandparents of children with disabilities had high risks of depression and thus required urgent attention. Healthcare providers and policymakers should develop and implement interventions considering intergenerational differences to reach optimal efficiency.

Multi-dimensional mismatch and barriers for promoting PrEP among men who have sex with men in China: a cross sectional survey from the Demand-side

by Yuan Guan, Tangkai Qi, Qibin Liao, Renfang Zhang, Jun Chen, Li Liu, Yinzhong Shen, Han Zhu, Qi Tang, Hongzhou Lu

Background: Men who have sex with men (MSM) is a key population for preventing HIV in China, yet pre-exposure prophylaxis (PrEP) is not widely accepted in this population. The objective of this manuscript was to assessed the barriers in the acknowledgement and uptake focusing the demand side. Methods: An online questionnaire survey was conducted from December 2018 to January 2019. All participants were required to scan two-dimensional code which was the online crowdsourcing survey platform to complete the electronic questionnaire anonymously. Results: Among 1915 MSM from thirty-four cities of China, 512 (26.7%) versus 1617 (84.4%) had an objective or subjective need of PrEP, respectively. One hundred and six (5.5%) reported affordability and only 23 (1.2%) had ever taken it. Age, living alone and occupation were associated with the objective needs. Age, income, sexual behavior were associated with actual usage. The participants who they had objective need to use PrEP are the population which we should focus on. Conclusion: A wide disconnect exists among the objective need, willingness, affordability and uptake of PrEP. Cost was the most prevalent barrier, accounting for 78.22% of individuals who needed and wished for PrEP but finally failed to receive it. The findings might facilitate optimizing future allocation of resources to better promote PrEP in Chinese MSM.

by Qi Tang, Jing Lu, Wenhui Wu, Zhenwei Liu, Sitang Zhao, Chengyue Li, Gang Chen, Jun Lu

Background: Polypharmacy has become a major and growing public health issue, with significant implications for health outcomes and expenditure on healthcare resources. In this study, a risk prediction model of polypharmacy represented by a nomogram for community-dwelling elderly patients based on the Chinese population was constructed. Methods: A cross-sectional study was conducted in Shanghai, China. The variables data affecting polypharmacy were fetched from the information system database of health government departments in Shanghai. The Least Absolute Shrinkage Selection Operator (LASSO) regression analysis was used to select the predictor variables, and multivariate logistic regression was used to establish the prediction model. A visual tool of the nomogram was established for predicting the risk of polypharmacy in the elderly population. In addition, the receiver operating characteristic (ROC) curve, calibration curve, and decision curve analysis (DCA) were used to estimate the performance of the model. Results: A total of 80,012 elderly patients were included in this study. Eight variables, containing age, residential area, preferred medical institutions, number of visits to tertiary hospitals, number of visits to secondary hospitals, number of visits to community health centers, number of diagnoses, and main types of disease, were included in the risk prediction model of nomogram. The area under the curve (AUC) of the nomogram was 0.782 in both sets, demonstrating that the model has a good discriminant ability. The calibration chart shows that the prediction model fits well with the validation set. DCA results displayed that the threshold probabilities of the two sets in the prediction model reached up to 90%, implying that the model had a preferable application value. Conclusion: This study explored the risk factors for polypharmacy among the elderly in Shanghai, China, and applied the nomogram to establish a predictive model via eight variables, which provided an effective tool for early screening and timely prevention of polypharmacy. Family physicians or pharmacists could scientifically use the tool to closely observe community-dwelling elderly patients, decreasing the adverse health effects caused by medication for the elderly.

by Qi Tang, Min Yuan, Wenhui Wu, Huanyun Wu, Cao Wang, Gang Chen, Chengyue Li, Jun Lu

For the disabled, paying attention to their health status is the starting point to discovering their survival problems, while meeting their care needs is the end point to solving their survival problems. As the country with the largest number of disabled elderly in the world, how to ensure this group could obtain appropriate home care is a major public health issue facing China. Thus, we conducted a cross-sectional study from October to December 2020 to explore the basic characteristics and health status of disabled elderly in different types of care who are living at home in 37 streets in Shanghai, as well as the individual care needs and its relevance. We observed the significant differences in the number of diagnoses (p = 0.03), smoking (p = 0.009), drinking (p = 0.016), exercise (p = 0.001), activity of daily living (p < 0.0001), and the quality of life (p < 0.0001) across care types. The care needs of the disabled elderly are diversified, of which a vast majority of them have not been fully guaranteed. The urgent need for improving the identification accuracy of care needs of disabled elderly, as well as the development of elaborate and personalized care programs for them, is needed.

by Qi Tang, Litao Wan, Jing Lu, Wenhui Wu, Huanyun Wu, Zhenwei Liu, Sitang Zhao, Chengyue Li, Gang Chen, Jun Lu

Background: As one of the countries with the most serious degree of aging, the incidence of potentially inappropriate drug use among the elderly is as high as 30. 4% in Chinese communities, and the lack of effective medication management and poor medication compliance at home are the main factors. Given these situations, we constructed a Rational Medication Management Mode based on family physician service, carried out an empirical research and evaluated the implementation effect. Methods: A prospective cohort study was conducted from September to December 2021 to analyze the implementation effect of the Rational Medication Management Mode by comparing the outcome indicators between the intervention group and control group. The primary outcome of this study was medication number and polypharmacy (taking 5 or more medications) at 90 days. The secondary outcomes included the situation for behavioral self-management and knowledge-belief-behavior of rational medication use. Results: A total of 618 elderly patients (309 in the intervention group and 309 in the control group) with multimorbidity were included in this study, those were all available at follow-up at 90 days. At 90 days, the number of medications was achieved by 3.88 (1.48), and patients with polypharmacy were reduced by 59.55% in the intervention group, having a significant difference compared with the control group (P < 0.001). Patients with medication reminders, intermittent medication and adverse drug reactions were achieved in 294 (95.15%), 47 (15.21%), and 51 (16.51%) respectively in the intervention group (P < 0.001). The knowledge, belief, behavior security and behavior compliance of rational medication use of elderly patients were all greatly improved in the intervention group at 90 days (P < 0.0001).Conclusion: The Rational Medication Management Mode based family physician service, which provides the support of manuals and pillboxes, can decrease the elderly patients' number of drugs with multimorbidity, reduce the incidence of polypharmacy, enhance behavioral self-management, increase the knowledge and belief of rational medication use, and improve the security and compliance of medication usage behavior. In order to provide a practical basis for rational medication management of elderly patients with multimorbidity under the background of long-term prescriptions in China.

Regional Disparities of Rehabilitation Resources for Persons with Disabilities in China: Data from 2014 to 2019

by Qi Jing, Qi Tang, Mei Sun, Xiaohong Li, Gang Chen, Jun Lu

Although the United Nations’ Convention on the Rights of Persons with Disabilities enshrines the right to health for all persons with disabilities (PDs), PDs face health disparities in terms of access to rehabilitation resources, which is important for service supply. This study aimed to explore the trends and distribution of rehabilitation resources for PDs in China from 2014 to 2019, explore the main factors that influence equity, and provide suggestions for policymakers. Data were obtained from the annual China Statistical Bulletin on the Development of Disabled Persons and the database of the China Disabled Persons’ Federation. Six types of rehabilitation resources were chosen to measure the trends in allocation and equity. Data on disparities were analyzed based on western, central, and eastern regions. The Health Resource Density Index and Theil Index were calculated to determine the degree and density of unfairness. The findings show a steady increasing trend in the amount of rehabilitation resources in China from 2014 to 2019. The density and equity of allocation of rehabilitation resources have improved greatly in recent years. Regional disparities were principally caused by differences within the regions. Suggestions including expanding investment in rehabilitation resources and developing rehabilitation systems were put forward.

  • A New Decomposition Algorithm of DCT-IV/DST-IV for Realizing Fast IMDCT Computation

by Hui Li, Ping Li, Yiwen Wang, Qi Tang, Lijian Gao

In this letter, a new decomposition algorithm of type-IV discrete cosine transform/type-IV discrete sine transform (DCT-IV/DST-IV) and architecture of a hardware accelerator for realizing fast inverse modified discrete cosine transform (IMDCT) computation are presented. The comparisons of computational complexity with some well-known algorithms show that the proposed algorithm possesses the advantages of higher computational efficiency and simpler hardware implementation. A real-time audio decoding experiment was performed to verify the efficiency of the proposed algorithm. Experimental results show more than one third of computational cycles are saved compared with one of reported fast algorithms for IMDCT computation.
